LFHC RECEIVES NCQA LEVEL 3 MEDICAL HOME DESIGNATION
Health care settings that facilitate successful partnerships between patients and their personal physicians
October 26, 2010, Brooklyn, NY
Lutheran Family Health Centers (LFHC) has officially received recognition as a Level 3 Patient-Centered Medical Home from the National Committee for Quality Assurance (NCQA).
Patient-centered medical homes, such as LFHC's primary care sites, strengthen the physician-patient relationship by replacing episodic care based on illness and patient complaints with coordinated care and a long-term healing relationship.
"Lutheran has long defined health as the total well being of the community and our residents," said Larry K. McReynolds, executive director, LFHC. "Medical homes, like our eight neighborhood based primary care sites, emphasize enhanced care through open scheduling, expanded hours and direct communication between patients, physicians and staff."
Aspects of care that must meet strict requirements to receive accreditation include access and communication, test and referral tracking, registry functions, care management, advanced electronic communications, patient self-management support, performance reporting and more.
The American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association have jointly defined the medical home as a model of care where each patient has an ongoing relationship with a personal physician who leads a team that takes collective responsibility for patient care.
The NCQA is a private, 501 (c)(3) not-for-profit organization dedicated to improving health care quality. The NCQA seal is a widely recognized symbol of quality. Organizations incorporating the seal into advertising and marketing materials must first pass a rigorous, comprehensive review and must annually report on their performance. Accredited health plans today face a set of more than 60 standards and must report on their performance in more than 40 areas in order to earn NCQA's seal of approval.
