- The Lutheran Family Health Centers network provides a high-quality and convenient health care safety net for neighborhoods throughout southwest Brooklyn. Our community health center network (FQHC) is one of the largest and most comprehensive in the nation. With more than 90,000 active patients, our network handles approximately 600,000 outpatient visits a year.
- Community-based services , including Adult and Family Education; Healthy Connections; AmeriCorps Community Healthcorps; Women, Infants and Children Program (WIC); Reach Out and Read; and Chronic Disease Management.
