What is a Medical Home? What is an FQHC?
LFHC is currently an inclusive array of nine primary care sites, 28 school-based health centers and dental clinics, three early childhood day care centers, two neighborhood centers for older adults and the nation's largest dental residency program and New York State's largest Behavioral Health Program. Lutheran's vibrant history, FQHC designation and unique Medical Home model are just a few of the factors that allow us to provide access to affordable, high quality and cost effective care to southwest Brooklyn.

The first family medicine residency program in New York City and one of the first in the nation was opened at Lutheran in 1973 by Eugene Fanta, M.D., (at right). Our dedication to family medicine is rooted in fundamental beliefs. Family medicine is the medical specialty best suited to meet the health care needs of individuals, families and communities over the course of lifetimes. Our belief is that health is a state of optimal physical, mental and social wellbeing, not merely an absence of specific disease. We believe in a continuous and comprehensive model of health care delivery that forms the basis of contemporary medicine.
What is an FQHC?
A Federally Qualified Health Center, or FQHC, is an organization that receives funding under section 330 of the Public Health Act. Lutheran Family Health Centers is the largest FQHC in New York and the second largest in the United States. As an FQHC, we are committed to serving a medically underserved population, offering sliding fee scale payments to make health care more affordable for the underinsured, and providing comprehensive and quality services. In addition, it means our governing board, the Lutheran Family Health Centers' Board of Trustees, has a consumer majority, where more than 50 percent of board members are users of the health centers as well as active members of our communities. This unique aspect of our board allows us to better address the needs of our patients and the community.
What is a Medical Home?
A Patient-Centered Medical Home provides each patient with a cohesive team of doctors that proactively work together with the best interest of each patient in mind to provide the best care possible. Unlike other health care facilities that often seem impersonal, a Medical Home enables patients to form a relationship with their physicians and specialists, fostering better communication and follow-up. Lutheran Family Health Centers has been designated by the National Committee for Quality Assurance as a Level 3 Medical Home, the highest level, in recognition of our high quality and patient-driven practice.
